User Experience Settings: Health Survey Configs

This article covers the settings in the Health Survey Configs widget on the User Experience page.  You can find information on the rest of the User Experience settings by clicking on one of the links below:

You can enable or disable the following options to display or hide them for users.  If you make a change to any of the options click the Save button at the top of the User Experience page.
Note

The User Experience settings are zone-specific. Ensure that you have the correct zone selected in Settings -> Zone Settings -> Current Zone before you make any changes to a setting.

Health Survey Configs

Feature Definition
Show Survey Option Requires users to take the Health Survey type selected.  

When disabled, any user who is fully vaccinated will have the health status of Low Risk

Survey Type Determine which survey to present to your users by selecting the survey type:
  • Full
  • Simple
  • Youth
You can view the questions for each survey type here.

Only one type of survey can be selected per organization. If you want to use more than one survey type we suggest creating a zone for each type.

Contact CLEARED4 if you need to create a custom survey.

If you select the Simple or Youth survey, change the warning score limit from the default of 55 to 45 to trigger the correct status. The  Warning Score limit is located in Settings -Thresholds

Forced Survey Refresh Date Forces all users to retake the full survey at a specific date and time, or select the days of the week you wish them to take the full survey. 

Tip: CLEARED4 will update the surveys according to CDC or government quarantine changes; users are then automatically required to retake the full survey once these changes go live.
Forced Survey Days Check the day(s) a user must take the Health Survey.

If the user does not log into the app on a day selected, they will not be required to take the Health Survey until the next required day unless  Strict Forced Survey Days is enabled.

When  Strict Forced Survey Days is enabled the user will be required to take the Health Survey as soon as they log in. 

Strict Forced Survey Days Forces users to take the Health Survey on the day(s) selected in the  Forced Survey Days field. If the user does not log in on a day selected, they will be forced to take the survey as soon as they log in. 

Repeat Survey Allowed Within Surveys taken within this time will not be recorded. This allows for correcting errors when the user fills out the survey.
